Body Evolution Official Rules
NO PURCHASE IS NECESSARY. ONE ENTRY PER PERSON PER CONTEST TERM.
TERM: This offer will last 30 days from the first day challenge commences each month.
PRIZES: Up to twenty (20) participants will be selected each month to receive an all-inclusive fitness package which includes RMR/VO2 test, 30 day cardiovascular program, 2 days/week small group personal training, 1 day/week yoga or Pilates session, 30 day meal plan, (4) 30-minute massages, (1) injury evaluation & prevention program, daily inspirations & meditations and access to the Fitness Evolution training facility (Approximate Retail Value "ARV": $1179); The specifics of all aforementioned elements of any prizes in the contest shall be solely determined by Fitness Evolution. No substitution, transfer of any prize (or any part of a prize package) or cash alternative is permitted.
ODDS OF WINNING: Odds of winning will be determined by the number of entries received. Up to twenty (20) entrants will be selected each month. Odds may differ based on participation.
HOW TO PARTICIPATE: Contestants must complete an "official entry form" provided at Fitness Evolution or any Body Evolution partnering location. All entries must be received by the 15th of each month to be considered eligible for the following month's challenge. Fitness Evolution and Lindora are not responsible for entries that are late, lost, illegible, misdirected, mutilated or incomplete, and reserve the right to modify scheduling of the contest without prior notice. Entries received after the 15th, or entries received after all slots are filled each month, will be held for the next available slot. Upon acceptance into the program, participants will be required to submit a $49 check to Fitness Evolution for administrative costs. This check will only be cashed should the participant fail to complete the 30-day Body Evolution program. Upon completion of the 30 days, the check will be returned or can be used toward any Fitness Evolution service.
NOTIFICATION: The prize entrants will be notified by email and/or telephone on or after the 16th of each month.
ELIGIBILITY: This contest is open Orange County residents age 16 years and older. Employees and their immediate families (spouses, parents, siblings, children and each of their respective spouses) of Fitness Evolution and supporting partner are NOT ELIGIBLE. Entry materials that have been tampered with or altered are void. All participants and/or entrants release Fitness Evolution and their agents, advertising and promotion agencies, affiliated companies, contest partners and prize suppliers, and all of their respective affiliated companies, employees, officers, directors and shareholders, from and against all claims and damages arising in connection with each entrant's participation and/or entry in the contest and/or their receipt or use of any prize awarded in this contest. Any and all warranties and guarantees are subject to the respective manufacturer's terms and conditions. This contest is governed by U.S. law and is subject to all federal, state and local laws. Void where prohibited by law.
CONDITIONS: Fitness Evolution, participating sponsors and organizations, officers, employees, contractors and agents are not in any way liable for damage, loss or injury from entering the contest or acceptance and use of the prize packages. The entrant must sign a liability waiver. By submitting an entry, participants accept and agree to these rules and the decisions of Fitness Evolution which shall be final. Entrants assume complete liability for injuries caused or claimed to be caused, by participation in this contest, or the use or misuse of any prizes. Fitness Evolution reserves the right at their sole discretion to suspend this program should causes beyond the control of Fitness Evolution corrupt the administration, security or proper play of the contest. Entrant may be required to execute an affidavit of eligibility and release, which must be completed and returned within ten (10) days of notification or an alternate entrant will be selected. Taxes, and shipping, handling and delivery fees, if any, are the sole responsibility of the prize entrant. By participating in this program, entrant grants Fitness Evolution permission for the use of his/her name and likeness for advertising and future promotion purposes without additional compensation except where prohibited. This challenge is subject to these complete and official rules. A statement of the entrants' names will be available after the 15th of each month. |
